Please review and accept the liability waiver before participating in our candle bar activities.

Posh Botanical & Grove Botany Boutique Liability Waiver

RELEASE/WAIVER: I hereby agree to indemnify and hold harmless The Posh B LLC and the Grove Marketplace LLC and its employees at, 301 E Jefferson St. Franklin, IN 46131, against all claims for personal injuries or damages of any kind arising from participation in any Apothecary or Candle Making workshop, specialty classes, and their resulting products.

*NOT MEDICAL ADVICE: I understand that no ingredients, ingredient descriptions, or guidelines presented are intended to treat, prevent, or cure any disease or condition.

*PHOTO CONSENT: By your signature, you agree that The Posh B LLC and the Grove Marketplace LLC may use the below named participants photograph in the routine promotion of its activities.

SUBSEQUENT VISITS: This waiver shall be kept on file and adhered to for both initial and subsequent visits to the workshop by the participant.

You may revoke this waiver at any time by sending a statement via certified mail to: The Posh B LLC Candle Bar located in the Grove Botany Boutique at 301 E Jefferson St. Franklin, IN 46131

I fully understand and acknowledge that the candle bar and workshops offered at The Posh B LLC and the Grove Marketplace LLC may involve inherent risks, dangers and hazards which may result in injury or illness. All Workshops offer a No Refund Policy.

I hereby voluntarily agree to release, waive, discharge, hold harmless, defend, and indemnify The Posh B LLC and the Grove Marketplace LLC, its managers, members, and employees from all claims, actions or losses for bodily injury, property damage, wrongful death, or otherwise, which may arise from my participation in the candle bar, any workshops or classes, resulting products from workshops or classes, or from use of equipment provided by The Posh B LLC and the Grove Marketplace LLC.

I accept full liability for creations made by the participant in the workshop.
